Patient Portal Registration Information
You will receive an email from CT Eye Physicians & Surgeons that invites you to register for the patient portal where you can review a summary of your office visits.
Please enter the below information ACCURATELY to register for the patient portal:
- NAME
- TEMPORARY PASSWORD – Enter 1234 as your temporary password. You can enter another one during the registration process
- INSURANCE POLICY NUMBER: If you do not have insurance you must enter your Social Security number
- EMAIL ADDRESS: This will become your Username
- DATE OF BIRTH: Format 00/00/0000
Once you have logged in, click “View Document” for your visit information.
